Urine test

If you suspect a urinary tract infection (bladder infection), you can leave a urine sample at the practice, together with a completed questionnaire.

Questionnaire

The urine test questionnaire can be completed at home and brought along, or filled in at the practice. The questionnaire is available at the entrance of the practice.

Submitting a urine sample

Place the urine sample together with the completed questionnaire in the cooler at the entrance of the practice. We would like to receive the urine sample BEFORE 10:00 AM.
