Medical records
1. Viewing your records
Patients have the right to view their medical records or to request a copy. This includes written information obtained from other treating physicians, except when this would violate another person’s privacy. The doctor’s personal working notes are excluded from the right of access.
After death
Next of kin do not have the right to view the records of a deceased family member. However, they can contact the GP to discuss their questions.
Making an appointment for viewing
Patients are advised to make a consultation appointment to view their records. The GP can then answer questions and provide explanations directly.
2. Copy of medical records
Copies can be requested from the doctor’s assistant. Provision takes at least one week.
Costs
- Up to 100 copies: EUR 0.23 per copy
- Maximum per request: EUR 4.50
Identification
Upon collection, you must identify yourself with a valid identity document (passport, identity card or driving licence).

3. Transfer of your records
When transferring to a new GP, the records are usually transferred. This is done by post or electronically via Zorgmail File Transfer (ZFT). Patients do not receive the original records but always have the right of access.
