Practice nurses (somatic)
General practices are receiving increasing responsibilities, partly because care is being transferred from hospitals to the GP and their staff. The number of elderly patients is also rising, as is the number of patients with chronic conditions such as asthma/COPD, diabetes and cardiovascular disease.
Practice nurses support the work of the GP and take over part of the chronic care from the GP. The GP and practice nurse maintain close ongoing consultation.
There are 2 practice nurses at Schoterpoort. In addition to their training as district nurses, they completed further training as practice nurses at Hogeschool InHolland. Diabetes and COPD care are their primary areas of focus. They also address health problems in elderly patients.
The practice nurse is of course willing to visit patients at home who, due to a disability or advanced age, are unable to come to the practice.
They work Monday to Friday with consultations by appointment.
